If you need to cancel your Surgery:

To cancel with My Lap Band Surgery Inc., you'll need to request a Cancel Surgery Authorization (CSA) number. This can be done by contacting our Accounting Department at mylapband@cfl.rr.com , or by telephone.

All Payments & Deposits are non refundable, all credit card processing and wire trasfer fees are the patients resposiblitiy. Always consult with your physician or other qualified health care provider before embarking on a new treatment, diet, or fitness program. You should never disregard medical advice or delay in seeking it because of something you have read in www.ObesityResourceServices.com or by the 3rd party advertisers.

- Clients name
- Invoice number if applicable
- Surgical Date
- Nature of the problem
